Students must complete a Student Immunization Form within the first 45 days of classes.

Two ways to complete the Student Immunization form: 

1. 在线
   • 登录 Student Immunization Records System with your StarID and password 
   • Follow the provided instructions on the form
   • Have your immunization information ready 
   • When you submit the form, your information will be electronically sent to the Immunization Coordinator on your behalf. 
2.  印刷形式
   • Download, print and complete the Immunization Form
   • 提交给St. 云州 Medical Clinic by either:
     • Fax completed document to: (320) 308-3192

If this is not completed, students will not be able to register for subsequent semesters.

Medical or Conscientious exemption: must use the printed form (option 2 above). 

*Please note that when you register do NOT register thru your cart, instead use the QuickAdd function in registration. Otherwise the classes in your cart will continue to block the registration even after the hold has been cleared.

明尼苏达州esota Law (明尼苏达州. 统计.135A.14) requires students enrolled in a public or private post-secondary school in 明尼苏达州esota be immunized against tetanus and diphtheria. 的 most recent vaccination must have been within the last ten years. 明尼苏达州esota Law (明尼苏达州. 统计.135A.14) also requires students be immunized against measles, mumps and rubella (MMR). All doses of MMR vaccine must have been received after the age of 12 months. Students are exempt from this law if they were born before 1957, graduated from a 明尼苏达州esota high school after 1996, or are enrolled in only one class or enrolled only in extension classes.

Your student immunization record only needs to be submitted one time within the first 45 days of your first semester.

Certain health profession academic programs request similar health information, but it is submitted elsewhere. Contact your academic program for information how to complete that requirement.
